Below are important forms and files for you and your camper(s) to look over, complete as necessary, and return to the camp office by the specified date.

Continuing with our mission of Going Green, we have decided to use as many electronic forms as possible. Many of these forms can be submitted electronically, and some will need to be completed, printed, signed, and mailed back to us.

We know this can be a little confusing the first time you do it. Please follow the directions for each form, and if you are having any difficulties, please do not hesitate to contact the camp office at: 978-402-2284.

Instructions: Open file, press “Highlight Fields”, complete the first three pages of the health form, press “Print Form”, sign under Parent Signature, have your child’s physician complete the last page (or attach their standard physical form) and mail it back to camp office by June 1, 2010.

If you would like to tell us a little more about your camper and their (and your) hopes for their summer camp experience, please use this form.

Instructions: Open file, press “Highlight Fields”, complete the form, press “submit by email” – if your email program does not automatically open up, please save the completed file on your desktop. Open your email program, create an email to camp@concordacademy.org and attach the file from your desktop. If you are having any difficulty emailing it, you may always print the completed form and mail it in. If desired, you can also email us a picture of your child, just make sure to name it with your child’s first and last names.

Open your email program, create an email to camp@concordacademy.org and attach the file from your desktop. If you are having any difficulty emailing it, you may always print the completed form and mail it in. You must also mail payment for extended day to the address below.

Please read this carefully (even if you have already attended camp!) This handbook should answer questions that you and your child have about camp this summer.

If your child is enrolled in Woodshop Camp either week 4 or week 7, please look over the available projects, and let the camp office know which one he or she would like to build. Please email or call the camp office with this information by June 1 so we can order the appropriate supplies.
